- Internal Medication (Shaman Chikitsa)
- Internal medicines are used to correct the deep-rooted imbalance (Dosha and Dhatu).
- Vata-Pacifying & Joint-Supporting Formulas:
- Guggulu Preparations: are classics for joint pain. Guggulu helps to clear Ama and balance Vata.
- Anti-inflammatory/Analgesic Herbs: Shallaki (Indian Frankincense) and Ashwagandha (an adaptogen that strengthens tissues).
- For Detoxification (Ama Pachan):
- For Ama, light purgation with Castor Oil (Eranda Taila) or formulations containing Triphala may be advised to cleanse the bowel, which is the seat of Vata.
- External Therapies (Snehana & Swedana)
- These are localised oil and heat treatments that help lubricate and strengthen the SI joint and surrounding tissues.
- Kati Basti (Oil Bath): Considered highly effective. Warm, medicated oil is held in a dough ring placed directly over the SI joint/lower back area for a specific period. This profoundly nourishes the area and pacifies Vata.
- Pinda Sweda (Bolus Fomentation):
- Elakizhi: Poultices of warm, medicinal leaves are used to reduce pain and stiffness.
- Shashtika Shali Pinda Sweda: A nourishing therapy using medicated rice cooked in milk and herbal decoction, excellent for strengthening the joints and preventing degeneration.
- Abhyanga (Warm Oil Massage): Gentle, daily self-massage of the lower back and entire body with warm Vata-pacifying oils to improve circulation and reduce dryness/stiffness.
- Panchakarma (Detoxification – The Cornerstone Treatment)
- Basti (Medicated Enema) is the principal therapy for Vata disorders and is often the main treatment line for chronic Sacroiliitis.
- Vasti/Basti: Since the colon is the main seat of Vata, herbal decoction enemas (Niruha Basti) and oil enemas (Anuvasana Basti or Sneha Basti) are administered. This procedure helps to bring the aggravated Vata Dosha back into balance, leading to pain relief and strengthening of the Asthi (bone) and Majja (bone marrow) tissues.
- Dietary Guidelines (Ahar Vidhi)
- Focus on Vata-Pacifying Foods:
- Warm, Cooked, and Moist: Favour soups, stews, well-cooked vegetables, and whole grains.
- Tastes: Emphasise sweet, Sour, and Salty tastes.
- Fats: Use liberal amounts of Ghee (clarified butter) and warm sesame oil for internal and external lubrication.
- Spices: Use warming, digestive spices like ginger, turmeric, cumin, and garlic.
- Foods to Avoid: Cold/chilled beverages, raw/dry salads, cold cereals, beans (unless well-spiced), and excessively spicy/pungent foods, as these aggravate Vata.
- Lifestyle & Yoga (Vihara)
- Rest and Posture: Avoid activities that worsen the pain, such as prolonged sitting, standing, or travelling on rough roads. Ensure proper, supported posture.
- Gentle Movement: Practice gentle, grounding exercises like light walking or yoga asanas prescribed by a therapist (e.g., knee-to-chest, gentle twists, cat-cow). Avoid overexertion or high-impact activity.
